Today’s content is told by Xu Mengyan, an outpatient nurse with many years of experience. During the period of breast-feeding clinic, she met many mothers to consult about the shortage of milk. Here, she shared several case studies with mothers to see why they were [short of milk].
Lack of consciousness leads to [insufficient milk]
Case: A mother gave birth to a baby girl 30 days ago, After giving birth, I have been feeding myself, Recently, the mother herself feels that the baby’s weight gain is not obvious, I always feel that my baby’s stool is obviously less, From several times a day in the past to 2-3 times a day now, The breasts will not feel as swollen as before, Worried about the poor growth of children, Afraid of not feeding enough, The elderly in the family also repeatedly urged her to add milk powder, However, Baoma still did not accept it very much in her heart. With puzzled eyes and a slightly anxious look, I came to my clinic. I noticed the child lying quietly in her mother’s arms. Very relaxed, Watching us talk with wide round eyes, Full skin, Flexible feet, In excellent condition, He also carefully inquired about the weight gain of the mother and child after each urine wet and the newborn physical examination. Through the inquiry, I got the answer: the child grew well and there was no so-called insufficient feeding. Looking at the mother’s breast again, it was true that the breast felt loose, but I pressed it gently on the areola a few times, and the milk slowly flowed out, smooth and natural, which was enough to make the baby eat and drink enough. So it was what who made her feel insufficient milk?
Analysis:
1. If milk does not rise, it means there is no milk?
There is a kind of milk rise called physiological period milk rise. Physiological period milk rise is commonly known as milk, which is reflected in the mother’s milk volume beginning to increase, and the breast becomes very full and swollen. This feeling starts 48 ~ 72 hours after delivery, and then with the balance of milk supply and demand, the feeling will be reduced. This is a natural phenomenon.
2. Is stool change insufficient feeding?
A week after the baby was born, There will be more than 5 urinations a day, It is also normal to have a large amount of defecation once or several times a day, Especially after the birth of the month, the baby’s stool habits will change. Some babies relieve it once every 2-3 days or even once a week. As long as the excreted stool is normal yellow soft stool, it is normal for the baby to defecate without pain and stool induration. As our common saying goes, it is called “saving stomach”.
3. Does the baby not gain weight?
The weight gain of newborns does not continue to rise from the first day of birth to the later. After birth, with excretion and consumption, the weight will decrease in a short period of time and basically return to the birth weight 7-10 days after birth. Then it will gradually rise.
4. Who should I listen to when I have a conflict of ideas?
As we grow older, there are fewer things we can be sure of, and so are parenting concepts. We are often influenced and confused by people around us, parents and so-called [experts]. At this time, we should calm down and see for whose good these are. Does this make sense?
Diagnosis result: It is not insufficient milk, it is a normal phenomenon.
Changes in the way of life and rest have led to insufficient milk.
Case: Mother B looked blurred, Entering the clinic with huge dark circles under his eyes, The spirit is slightly tired, He is thin. Asked why he went to see a doctor, Baoma explained, Knowing that breastfeeding is good, I insist on breastfeeding, At that early stage of discharge, The baby can sleep quietly after breast milk, But recently, soon after the baby is fed, The baby was in a hurry to find milk again, Even crying, Baoma can only continue to nurse, Frequent and almost uninterrupted feeding led to Baoma’s failure to have a good rest. Children also have short sleep time, Apparently feeling tired, Psychologically tired, I think nursing is an endless thing, In recent days, I feel that the amount of milk has obviously decreased, Before, I could squeeze 30 ~ 50 ml of milk after eating, but now I have no more milk to squeeze. I feel that breast-feeding seriously interferes with my life. I observed the accompanying babies, It is true that compared with ordinary children, The child is slightly thin, The mental state is not bad, Asked about the baby’s weight gain, The child has been 50 days old, But she only gained about 1 kg. Asked her mother about her diet, She has always been fond of a light diet, Very little greasy food is eaten, Mainly vegetarian dishes, It is also the basic three meals a day, There are few extra meals. I looked at her milk, Gently press the breast, Found that the milk was thin, My mother told me that there was basically no thick milk flowing out. Every 20 minutes of feeding, I feel that I have no milk and it is over. I have never felt the blockage of milk lumps. Through inquiries, I know that the irregular diet structure and work and rest of my mother have led to the fatigue of feeding. Changes in my body have affected my psychological emotions and indirectly led to the lack of milk quality and quantity. This is a process that needs systematic adjustment.
Analysis:
1. Judging milk mainly depends on whether the baby has enough to eat.
Here are a few tips: Tip 1: Urine volume: The baby does not urinate more than 6 times a day. Tip 2: Mental state: The baby should respond positively and sensitively. Tip 3: Sleep condition: Between two breastfeeding, the baby feels very satisfied, sleeps soundly and quietly. Tip 4: Pay attention to the growth curve: The younger the child is, the faster the weight gains. The following is the growth curve:
The five lines in the figure are 97%, 85%, 50%, 15% and 3% respectively from top to bottom, which represent the five grades of children’s development, regardless of good or bad, and it is normal to be in the middle.
However, looking back on Case 2, the slow weight growth of the baby and the lack of satisfaction after sucking may indicate that milk is insufficient.
2. Why is there insufficient milk?
There are many reasons for the lack of milk. But case two, We can find that psychological factors are very important. Stay happy, Emotional changes cause hormone levels to change, Thus affecting the secretion of milk, Ensure adequate rest: Adequate and high-quality sleep can promote milk secretion, Although it is inevitable that nursing at night will affect sleep, However, we can sleep when the baby sleeps, feed the baby when he is hungry, and try to rest synchronously with the baby. 3 Master the correct posture: effective sucking can promote milk secretion, but this not only refers to the number of sucking, but also the quality of sucking, do not change the nursing posture, find the most comfortable point to get twice the result with half the effort.
3. How to deal with psychological fatigue?
We can do what we like, let our emotions be vented, and we can also find friends and husbands to talk to. In a word, don’t be alone and don’t think about it all the time. Here, we should also remind everyone that breast-feeding is definitely not a matter for wet nurses alone, and we need the common persistence of our family.
4. How to eat a balanced diet during lactation?
Keep the food fresh and balanced, Reasonable intake of nutrition, Is the best way to eat. At the same time, Milk is mostly made of water, protein, fat, Proper consumption of lean meat, fish, fresh vegetables and fruits and calcium-rich foods will be very helpful. Mothers still need to increase their intake of some additional nutrients for the quality of breast milk. At the same time, you need to eat some fat-rich foods. If you feel greasy in your diet, you can add some vegetables to meat dishes, which is nutritious and tasteless.
Is it nipple depression that is not well connected?
Case: Mother C is tall, She also has big breasts, but she is also confused by small troubles, Her nipples are not protruding from her breasts as obviously as other mothers. She is a little unsure, It takes a lot of time to connect each time, He kept stuffing it into the baby’s mouth, But babies often can’t catch them at one bite, The feeling effect is not ideal, I also feel my back ache, And feel that the breast as a whole is in a soft state, No type, Also worried that the baby would not eat enough. After observing the mother’s breasts, It is too big, But I pulled my nipple lightly with my finger, It immediately protrudes from the breast, The shape of the nipples is also acceptable. So I let my mother sit in a comfortable position. In the semi-lying recumbent position, Relax, Hold the baby, Let her lie down on her mother’s abdomen, Headrest in the mother’s chest, let the mother hold the baby’s buttocks and chest back, the baby was lying quietly in the mother’s arms, suddenly, she raised her head, quickly looking for the mother’s breast, a sharp plunge into the big breast, quickly completed the holding action, satisfied and powerful sucking up. Mother was surprised to find that it became so easy to nurse.
Analysis:
1. Is the nipple really sunken?
Some mothers’ nipples may not protrude obviously from the breast, but this does not mean that they are sunken, which only accounts for 3%. We can make appropriate corrections at the 37th week of pregnancy, such as Hoffman’s fingers exercise or nipple orthosis.
2. Can I help with the breast-feeding by changing the breast-feeding posture?
For those with large breasts and those with difficult connection, we can try semi-lying feeding to reduce the area of breasts by gravity, which is conducive to the baby’s active breast search and connection.
3. Can sucking change the shape of nipples?
A few days after the baby passes the effective feeding, the mother will find that the nipple is lengthened, and the feeding of the baby is not as difficult as in the previous period, because when the baby sucks, it not only contains the nipple but also includes most of the areola, which also effectively helps the mother’s confidence in breast feeding.
Summary:
No one is perfect, it is these imperfections that make us different and lovely, believe in ourselves, strive to overcome these difficulties and become a better self.
